You may check in from anywhere at anytime, on the same day of your appointment.

Using the link sent to your email and cell phone at 7am on the day of your appointment, avoids having to enter the SSN/PIN and DOB.

If your appointment is being held by telehealth, please enter the address where you will be physically located at the time. For example, the address of your home, office, hotel or out of town residence.

During your appointment, you must be physically located within the state of Delaware or Maryland.

If you are in a vehicle and you are the driver, it must be parked during your appointment.

If you are going to be seen in the office or located at the home address on file, you don't have to enter the address, just select the appropriate option below.
