Portal Setup and Forms Completion
We recommend using a computer instead of a mobile device for this process if
possible.
If you must do it on a mobile device, note that some of the things you need to
click on might not be visible if you are holding the phone upright. Try turning
your phone on its side and this should correct the issue.
For the initial login you will use your email as
the username, and the provided password. Keep in mind that for
increased security, the password is case sensitive.
When entering the password you may receive an error
message if you copy and paste it because you may include leading or trailing
spaces. In this case type it in yourself instead of copy and paste.
The user name can be changed when logging in for the first time, and can be changed at any time.
The user name must meet the following rules:
1.
It must be at least 12 characters long and no more than 80 characters in length.
2.
It is recommended to use symbols and number, but it is not required.

If you still need help logging into your portal, please contact us.
After opening the portal web page, log in using the above information. Notice
the menus on top that opens by clicking on your name, Reports, or Accountings. Follow the steps below to complete your “paperwork”.
1.
After logging in you will see your My Profile.
To make
changes
to your name, address, emergency contact,
etc. click on the green Revise
button located near the top of the screen. Please complete as much
information as possible. After changes are submitted, the green Revise button
will turn into a red Pending Review button. You can click it again to make
additional changes to your demographics. If you are living at school, please
provide your address at school.

Click on your name then My Signature.
Using your mouse, draw your signature.
Don’t worry about perfect penmanship. The signature in a
document will always
include your printed name and other information.
Click on “Sign and Save”. There will be a about a 20 second delay to allow the system to upload and save the signature.
This is the equivalent of your hand written signature, and will
be used for all forms you electronically sign.
3. Click on your name then My Documents. You will notice a button titled Select Documents. Click on this button to complete and sign them. Only documents that have not been already approved by our staff are listed. Notice there is a “Patient Signature” section at the bottom of each document. Unsigned forms are incomplete and cannot be accepted. To sign a form click on the “x” next to Patient Signature, your signature should appear, and then click on Save. Then click on Send for Review. If the signature does not appear try doing step 2 (above) again. This will submit your document for review by our staff. After a document is approved, the status will change from In-Review to Locked. Until a document is Locked, you may edit or delete it. Documents that are not submitted are not seen by our staff. You may also download the signed forms at anytime for your records by clicking on Reports and Download Charted Documents.

Please remember to Sign, Save, and Submit for review.
To schedule your first appointment:
Click on your name, then My Appointments
Click on Schedule New Appointment
Select New Patient for the Visit type (Note
that the first appointment will be 1 hour even if you were seen
in another practice.)
Click on Openings (You can search for an appointment up to 180
days in advance)
Select the date and time you prefer
Click Save
Your appointment request will show as Pending (It will not be
Approved until all
the forms listed above have been signed, submitted, and
approved)
Note, appointments scheduled through the portal are pending until approved by
our staff. Please don’t assume a pending appointment is Approved, even if you
get a reminder email. Check the portal to be sure.
If you are being seen via tele-psychiatry we use the Doxy.Me platform. This service requires the use of the Firefox or Google Chrome browsers, a camera, and microphone. Visit Mark deBussy, APRN and click on the "Pre-Call Test" button to see if your computer or phone is compatible with this service.
